TERMS AND CONDITIONS: Promotion is valid for rebate up to a maximum of $2,000 from any combination of the following Schwan’s Food Service products listed on Rebate Form: 56711, 69100, 69151, 69039, 70249, 70250, 70256 and 69049 (Eligible Products). Participants must sign-up to participate in this promotion between 11/15/2024-3/31/2025 by completing this form and submitting prior to purchase. A submitted sign-up form is required for participation. Participants must purchase the minimum number of cases as specified above of an Eligible Product within the promotional window (1/1/2025-3/31/2025) to qualify for estimated rebates. Participants are NOT eligible for the rebate under this promotion on any of the Eligible Products of which the participant is already stocking open coded and of which the participant has purchased an average of 20 or more cases per month in the six-month period prior to the promotion window. Purchased products must be stocked open coded. Participants must have zero case volume for each Eligible Product in the prior six-month window from the promotion window to be eligible for payout. Offer is available to C-Store distributors only. Participants are responsible for freight and administrative costs on purchases. Request for rebate and submission of invoice copies is not necessary. Payment will be issued based on purchases of the selected product(s) within the promotion window. Please allow 4-6 weeks for payment processing following the completion of the promotion. Deductions, under any circumstance, are not allowed for the program and will be re-billed. The price of Eligible Products may change during the promotional window.
